Nutrition Assessment, Monitoring and Evaluation


Nutrition Assessment 
Individuals at risk of or with pressure injuries should be screened for malnutrition and/or have a full nutrition assessment conducted by a dietitian. The nutrition assessment of an individual with pressure injuries or diabetic foot ulcers should help to identify undernutrition and may include the parameters using Nutrition Care Process (NCP) terminology in the table below.
